Test schedule:
The three tests and the final will be common to all sections of Math
304 this semester. None of the tests will be in your regular class room,
or at the regular class time. Here's the schedule:

There will be no make up tests except for documented conflicts with the test time, or extraordinary circumstances beyond your control. See the paragraphs below labeled "Test conflicts", "Students with disabilities", and "Illnesses and emergencies".

Test conflicts: If you have a conflict with the scheduled test time then you can request an alternate test time or a makeup. You must make your request by the Friday before a test, or by the Thursday before the final exam.
Acceptable conflicts include other regularly scheduled classes and scheduled work time. Recreational activities do not count.
The University defines a conflict for a final exam to be another exam at the same time, or a total of three exams scheduled within the same 24 hour period. Nothing else counts. The same rules apply for scheduling a makeup final. We will only look at your exam schedule in other courses when deciding when to schedule a makeup final exam.
The only way to request an alternate test time or makeup is to fill out the test conflict form. Do not just ask your instructor for an alternate test time or makeup; all decisions will be made based on the test conflict form.
Students with disabilities: We are committed to full and equitable access for all enrolled students. To request special test accommodations based on a disability you must register with Services for Students with Disabilities. If you bring an Academic Accommodation Memo from Services for Students with Disabilities to your instructor then you will receive appropriate accommodations on all graded course quizzes and tests.
You must also follow the instructions above (under "Test conflicts"). Indicate in the "Comments" box of the web form the accommodation that you require for the test. Be specific; for example, let us know whether you are entitled to 50% more time, or to double time.
Illnesses and emergencies: In case of illness or an emergency (medical or otherwise) that will affect your ability to take a test you must inform your instructor before the test. If an unanticipated emergency arises immediately before an test that affects your ability to take the test, then you must either send email to your instructor before the test explaining your situation, or you must alert your instructor to the situation as soon after the test as possible. You must present a record of medical treatment or police involvement in such an emergency.

These tests are presented only as extra study material, to illustrate the type and level of question that may appear on tests this semester. You should NOT expect the questions on your tests to be restricted to the precise type of questions that you find on the samples.
